How does hair loss happen?

There is an enzyme in the body called 5-Alpha-Reductase (5AR), which converts the hormone Testosterone into another hormone - Di-Hydro-Testosterone (DHT). The DHT is produced internally inside the body and also topically on the scalp.

( DHT which is formed inside the body is produced from the 5AR enzyme - Type 2. The DHT that is formed topically on the scalp is converted from the 5AR enzyme - Type 1. ) 

This DHT finds its way to the scalp hair follicles and then plugs into specific points inside the hair follicles, called Androgen Receptor Sites.

Once there, the DHT causes a change in the growth pattern of the hair follicles, so that they start to grow smaller. The result is that the hair strands inside those affected hair follicles also grow smaller, thinner and weaker. Sometimes those hairs also grow lighter in color. At the same time, the blood vessels leading to the roots of those hair follicles also tend to shrink in size.

Normal Hair Growth

Scalp hair typically grows for a period of two to five years at a rate of approximately one half inch per month. After a ninety day resting cycle new hair replaces the old one which has been shed.  In people who are experiencing pattern hair loss, there is a change in the cycle of hair growth. Growing cycles become shorter and resting cycles become more frequent.  A negative change in the caliber of the hair may also be indicative that one is experiencing the effects of male or female pattern hair loss.

Hair Growth Stages

Growth Stage (Anagen)
Normal healthy hairs grow at the rate of about 1/2 inch per month, but vary by individual. A healthy hair follicle produces a strong, thick, colored hair. The hair follicles grow the hair for a period of 2 to 6 years in increase in diameter during this phase. In a normal scalp, about 90% of the hairs are in this stage at any given time. Some follicles produce thick colored hairs, called terminal hairs and other follicles produce short, thin, non-colored hairs, called vellus hairs.

Transition Stage (Catagen)
After the growing stage, the hair goes into a short transitional stage, which typically lasts about two to three weeks. Typically less than 1% of your hair is in this stage at any given time.

Resting Stage (Telogen)
Next the hair follicles enter a period of rest for approximately three months. After that time, the follicles lose their old hairs and resume the growing stage of producing new hairs. About 10% of your hairs are in the resting stage at any given time.

Most individuals lose between 40 and 100 hairs per day. This is a very small amount of hairs, considering that the average head of hair has approximately 100,000 individual shafts of hair.

Normally, as the hair goes through these three stages, the follicles tend to return to the same length and width during the growing phase. As a result, they generally produce the same length and width hair shafts.

All hairs cycle through these three stages repeatedly. The length and thickness of each hair shaft is determined by how long the hair itself is allowed to grow in the follicle before entering into the resting / shedding stage. Consequently, pattern hair loss is actually a gradual conversion of thick terminal hairs to thin vellus-like hairs.

Why am I losing my hair?

Due to the continued ever-increasing effects of DHT in the scalp hair follicles, the Growth Stage is getting shorter, while the Resting Stage is getting longer. 

Consider an analogy of leaves on a tree. Picture a situation where the Winter or Resting Stage gets longer and longer, while the Spring or Growing Stage gets shorter and shorter. This is what happens when you are in the process of losing your hair.

This pattern of male or female hairloss generally occurs over a period of several years to many years. All the time, DHT continues to be produced inside the body and topically on the scalp, and then lock inside more hair follicles in the scalp. Eventually the hairs may simply not grow back at all and they just fall out. So by the time that you notice an area of baldness on your scalp, it means that this pattern hairloss progression has been happening already for many months or possibly several years.

The more that DHT accumulates and locks inside the scalp hair follicles, the more the hairloss occurs and the faster it occurs. Once DHT locks inside the hair follicles, it cannot be removed. So you have to stop it, that is, block the DHT, before it gets to the hair follicles.

Does Minoxidil really work to stop my Hair Loss?

Minoxidil does not contain ingredients which block DHT from getting into your hair follicles. Actually it was developed many years ago to treat high blood pressure. It was discovered that some individuals using the drug had a temporary or slight decrease in hair thinning or hair loss. So it was put into a liquid form that could be used externally right on  the scalp and marketed as Rogaine.

So when applied topically to your scalp, Minoxidil is simply designed to dilate the blood vessels in the hair follicles that are growing smaller, with the idea that more nutrients might be able to get to those hair follicles to help keep them alive. However, it is essentially not blocking DHT and consequently, more and more DHT still gets inside the hair follicles, which increases the hair thinning and hair loss process.
